Time is running out to complete your online health assessment and Wellness Engagement Option. Employees and covered spouses are eligible to save up to $900 discount on your 2020 medical plan. Employees can save $600 and employees who also have a covered spouse can save $900.
To qualify for the discount, employees and covered spouses must reach 100% of the Wellness Program as reflected on your Win For Life dashboard on mycigna.com. To reach 100%, you will need to complete your Annual Physical, your Health Assessment (on mycigna.com) and one Wellness Option. Be sure to use biometrics (from your doctor) from February 1, 2019 or later. Don’t choose answers like “I don’t know” for your biometrics and answer all the questions.
- Annual Physical Exam – 10% Obtain your Height, Weight, Blood Pressure, HDL/Total Cholesterol and Waist Circumference from your doctor.
- Online Health Assessment – 10% Use your biometric measurement numbers to complete the Health Assessment on your mycigna.com
- Wellness Engagement Option – 80%
Complete one wellness engagement option by noted deadlines:
Option A: Preventive Care (February 1, 2019 – January 31, 2020) DEADLINE HAS PASSED
Option B: Telephonic Coaching (March 10, 2019 – March 13, 2020)
Option C: Onsite Programs (May 1, 2019 – March 13, 2020)
Option D: Achieve Health Goals (February 1, 2019 – January 31, 2020)
Option E: Tobacco Cessation Program (May 1, 2019 – March 13, 2020)
